Topical PGA such as Latanoprost, Tafluprost, Travoprost or Bimatoprost which is a synthetic prostamide are drops use to lower IOP. PGA is now recommended as first line of treatment with patients of IOP of 24mmHg or over who are risk of visual impairment in their lifetime.
So to IP students: Would you prescribe PGA to a 92 yo patient who presents to your practice with an IOP of 28mmHg, CCT 580 with minimal field defect and angles open on gonio? Px is asymptomatic with BCVA of 6/9 BE.
A bit of history:
Latanoprost was introduced in the US and Europe in 1996 and is now the mostly widely used PGA and ocular hypotensive drug in general. It reduces IOP by increasing uveoscleral outflow and current optimal dose reduces IOP by about 30% from baseline.
